If you suspect your child has a concussion, please contact your child's Vice Principal. Please review and access the forms below ensuring the forms are submitted to your child's Vice Principal.

Concussions can be a significant concern for student health and well-being, and a challenge to learning. The Thames Valley District School Board is committed to protecting the safety of our students as they engage in all school activities. This commitment includes promoting concussion awareness, and following procedures for the prevention of concussions while at school, and for responding to suspected incidents of concussion to ensure safe and healthy return to learning and physical activity. School staff, students, and caregivers all have roles to play in promoting safety. Below are resources to provide information on concussion signs and symptoms, and on TVDSB's concussion protocol and return-to-school and return-to-sport processes.
